Making a Referral to House2Home

House2Home works in partnership with Local Authorities, charities, and support agencies to provide safe, supported accommodation for people experiencing homelessness and those at risk of homelessness.

Our referral process is designed to be simple, responsive, and transparent.

Who Can Make a Referral

Referrals are accepted from:

  • Local Authorities
  • Charities
  • Housing teams
  • Support agencies
  • Other third-party organisations working with individuals experiencing homelessness

Referral Criteria

All referrals are assessed on a case-by-case basis to ensure the placement is appropriate for both the individual and the accommodation available.

Referrals should be for individuals who:

  • Are aged 18 or over
  • Are experiencing homelessness or at risk of homelessness
  • Would benefit from supported accommodation
  • Are willing to engage with support

Exclusions

Any exclusions will be assessed individually and discussed openly with the referring agency during the referral process.

Information Required

To help us assess referrals quickly and accurately, we ask that the referral form is completed in full.

This typically includes:

  • Personal details
  • Current housing situation
  • Support needs and risks
  • Any relevant agency involvement
  • Emergency contact details

Step-by-Step Referral Process

1. Download and complete the referral form

2. Submit the completed form

3. Referral assessment

Once received, the referral will be reviewed by a member of our support team.

4. Initial decision

We aim to review all referrals within 24 hours.

5. In-person assessment

If the referral is initially accepted, an in-person assessment will be arranged to ensure suitability.

6. Placement

Where appropriate, a placement will be offered subject to availability.

What Happens Next

Once a referral is accepted:

  • A support plan will be developed based on the individual’s needs
  • Ongoing support will begin immediately following placement
  • We maintain communication with the referring agency where appropriate

Timescales

Referral review: Within 24 hours

Assessment: As soon as possible following initial acceptance

Placement: Subject to availability and suitability
