Person-Centred Supported Housing Services

At House2Home Supported Housing CIC, we provide needs-led supported housing services for people experiencing homelessness and those at risk of homelessness.

Our support is built around the individual — recognising that everyone’s situation, challenges, and goals are different. We focus on stability, independence, and long-term outcomes, not quick fixes.

Who We Support

We provide tailored support to a diverse range of residents, ensuring that everyone, from single adults to families, receives the guidance and accommodation they need to build stability and move towards independent living.

Our services are available to adults aged 18+, including, but not limited to:

  • Single adults (male and female) experiencing homelessness
  • Individuals at risk of homelessness
  • Asylum seekers
  • Prison leavers
  • Families and couples

Our Approach

Needs-Led, Flexible Support

Each resident receives a minimum of 1–2 hours of in-person support per week, with additional support provided where required.

Support at House2Home is:

  • Ongoing – not time-limited
  • Tailored – based on individual needs and circumstances
  • Person-centred – shaped around the resident, not a fixed programme

Types of Support We Provide

All support plans are reviewed regularly and adapted as residents progress.

While every support plan is different, our services typically include:

Tenancy Sustainment

  • Support to understand tenancy responsibilities
  • Help maintaining accommodation and communal living
  • Encouragement to build positive routines

Independent Living & Life Skills

  • Budgeting and managing household finances
  • Developing daily living skills
  • Support with appointments and organisation

Housing & Move-On Support

  • Planning for future independent accommodation
  • Support with applications for move-on housing
  • Liaison with Local Authorities and housing providers

Multi-Agency Working

  • Working alongside Local Authorities
  • Coordinating with third-party support agencies
  • Supporting individuals involved with probation or asylum services

Health & Wellbeing Signposting

  • Support to access GP services and healthcare
  • Signposting to mental health services
  • Signposting to substance misuse support where required

How Support Is Delivered

There are no fixed timeframes for support. Residents move at their own pace, with the ultimate aim of sustaining an independent tenancy when ready.

Support is delivered through:

  • In-person sessions at the property
  • Ongoing communication with partner agencies
  • Regular engagement to monitor progress and wellbeing

Our Outcomes

Our long-term goal is simple:

To support residents to move on and successfully sustain their own accommodation.

We work towards this by:

  • Reducing repeat homelessness
  • Increasing housing stability
  • Supporting residents to build confidence and independence
  • Ensuring accommodation is safe, stable, and well managed

Quality & Compliance

We are committed to delivering high-quality services and:

  • Adhere to Regulator of Social Housing standards
  • Ensure all properties are safe, compliant, and well maintained
  • Carry out weekly property inspections through our in-house maintenance and compliance team
